For 9th-12th Grades, as of September of each year. Earn a High School Letter for service!

   2011/12 Varsity Letter in Community Service - Application

   Due by April 1, 2012 to be considered for a letter award. Students performing ANY amount of volunteer hours, from April 1, 2011 - March 31, 2012 should complete and return the application to United Way. All applications received by the deadline will be invited to the Youth United Recognition ceremony

   This is the electronic version of the above form.  You may fill it out on your computer and email it back to us, rather than print it and hand fill it.

   Use these forms to document your volunteer projects. Remember to use a separate form for each project, unless it is for the same organization or supervisor. Same supervisor/organization, same form. Different supervisor/organization, different form.
